Community Member Well-Being (CARE)
This category is for sharing concerns about a student. Occasionally, members of our community find themselves or others in need of additional help and support. Examples include: Struggling academically, Mental health problems, Physical health problems, Family crises, Personal challenges, Academic or work absenteeism, and Substance use problems or concerns.
Examples of what to report
- Someone who is usually socially active becoming noticeably withdrawn
- Uncharacterstic or unhealthy mess in personal spaces such as a dorm room or office
- Use of illegal or non-prescribed substances
What happens after you submit a report?
This form is sent to the UVM CARE Team, who's CARE Team Outreach Coordinators begin a response by gathering more information and reaching out to the student to offer support. How the CARE Team reaches out depends on the content of the referral and the assessed level of concern. The response can range from an email or phone call to the student offering support to visiting the student in their room or finding them on campus to assess safety and immediate concerns. This category is for reporting campus security issues, especially campus crimes, Clery Reports, hate crimes and hazing incidents. Reports can be made by calling Police Services directly at 802-656-3473 or by text through the CATSafe App (powered by LiveSafe), as well as through the Crime Report Form (button below). All reports made to UVM Police are automatically classified as Clery Reports when appropriate.
A special note about hate motivated crimes. A hate crime is a criminal offense against persons or property that is motivated by the victim’s actual or perceived membership in a protected category. Hate crimes are defined by Vermont law at 13 V.S.A.§1455. Again, report to UVM Police at 802-656-3473 (or 911 if an emergency exists) as a first step. What happens after you submit a report?
These reports go to UVM Police Services where they will be reviewed, and an appropriate response will be determined, which may include dispatching UVM Police Officer(s). All on-campus crimes and security concerns should be addressed by calling Police Services directly at 802-656-3473 or by text through the CATSafe App (powered by LiveSafe), where dispatchers can assist you with reporting the incident.

This category is for identifying and removing barriers to access for students, faculty, staff, and visitors to the UVM campus. Additonally, this report is related to compliance with U.S. laws and regulations relating to the provision of equal access to those with disabilities, and to provide reasonable and effective accommodations that enable equal access to UVM's programs, services, activities, and information.

What happens after you submit a report?
Reports are reviewed by the Office of Accessibility Services (OAS). Depending on the nature and urgency of the concern, OAS may coordinate with other university offices, such as Facilities Management, University Housing and Dining Services, Enterprise Technology Services, or other responsible departments to address the barrier and provide updates as appropriate.

Equal Opportunity (Sexual Misconduct, Discrimination, Harassment, and Bias Concerns)
This category is for situations involving protected categories including race, color, religion, disability, national or ethnic origin, age, sex, veteran status, gender/gender identity, and sexual orientation. Anyone with concerns about discrimination, harassment, or bias involving a UVM community member or program is encouraged to report and can contact The Office of Equal Opportunity at 802-656-3368 or EqualOpportunity@uvm.edu at any time. UVM encourages students, staff, and faculty who have experienced an incident of bias, discrimination, or harassment, including sexual misconduct (sexual assault, relationship violence, gender-based stalking, sexual exploitation) to report the conduct to UVM.
If you are a Mandatory UVM Reporter, you must report all incidents of discrimination, harassment, and sexual misconduct, involving a UVM affiliate (student, staff, faculty, other) or occurring within the context of a university program using the reporting form.

What happens after you submit a report?
Once a report is submitted, staff in The Office of Equal Opportunity will provide information about support resources and options for investigative and restorative resolutions to empower those involved to determine the path that’s best for them. Supportive measures are available regardless of an individual’s desire to pursue a resolution process.
Anonymous or incomplete reports may receive a limited response, and those reporting incidents are encouraged to share as much information as possible. Information is shared with other University officials only when necessary, such as when community safety is impacted or legally required.

Ethics and Compliance (Financial misconduct, retaliation, misuse of personal data)
This category is for reports directly involving compliance with regulatory requirements, laws, and policies. The Ethics and Compliance Reporting Helpline allows UVM employees (faculty, staff, administrators, temp employees, and graduate students) to report violations to UVM’s Code of Conduct and Ethical Standards, UVM Policies or Procedures, as well as any other compliance concerns. The Helpline is also a place to seek guidance on these issues without fear of retaliation. The portal has a branching logic system and can help to identify a broad range of issues such as financial misconduct, legal noncompliance, conflicts of interest, or unauthorized use of patents or trade secrets.

- You overhear a manager talking about how they recieved a personal check for $2,000 after UVM signed with a new vendor
A non-affiliated organization or individual is using UVM branding to sell unlicensed products
A recently terminated employee refuses to turn in equipment such as a laptop or hard drive with potentially sensitive data
What happens after you submit a report?
In order to maintain the highest level of confidentiality, the University has contracted with an outside entity to manage the Compliance and Ethics Reporting and HelpLine. The outside entity handles the intake of all reports made via the HelpLine, screening them for conflicts before forwarding them to the Asst. Chief Safety and Compliance Officer, who reviews them with the Chief Internal Auditor and the General Counsel to determine an appropriate course of action. If an internal referral is needed, it follows a predefined referral matrix aligned with University procedures. For external referrals, the Asst. Chief Safety and Compliance Officer coordinates the referral through the Office of General Counsel. Investigations result in a corrective action plan based on the issue and findings. Once fully investigated, the report is closed. As long as you retain your login information, you can anonymously track the status of their report, including receipt, referral, and closure.

Student Conduct (Behavior and substances)
This category is for potential student violations of University Policy, including violations regarding Alcohol, Cannabis and Other Drugs, Hazing, Discrimination Harassment and Sexual Misconduct, Student Organization Misconduct, and Academic Integrity. These types of reports should be made within 24 hours of the incident and cannot be anonymous.
Examples of what to report
- A club won't let new members join unless they "prove" themselves by spray painting the water tower at night.
- A student is growing a cannabis plant in their dorm room
- A fight breaks out behind a residence hall. No one calls the campus authorities, but you know who was involved.
What happens after you submit a report?
These reports go to the Center for Student Conduct and the full details of the report will be shared with the student(s) against whom the allegations are made, including the name of the person reporting, the text of the report itself, and any attachments of additional evidence. A conduct report usually results in a Conduct Meeting between the student(s) whom the report has been made against and a meeting facilitator from the Center for Student Conduct.

Academic Integrity (Cheating, plaigarism, collusion)
This category is for suspected violations of the Code of Academic Integrity, including plagiarism, fabrication, collusion and/or cheating. This form is usually completed by member of UVM faculty. Any information submitted will be made available to the student(s) referred. If there is information you would like to share with the Center for Student Conduct that you do not wish to share with the student, please email sconduct@uvm.edu directly. Any information from an anonymous source will not be considered relevant and cannot be considered in the determination of the outcome of the case.

- Plagiarism or obvious AI prompts left in a final paper or assignment
- A student copying answers from another student during an exam
- Multiple students submitting the same assigmment under different names for a non-group project
What happens after you submit a report?
The report will go to the Center for Student Conduct who review the information and schedule a meeting with the students invovled. The student(s) may choose to accept responsibility for the violation and agree to sanctions or choose to go forward to a formal Academic Integrity Council meeting to determine the outcome. Incident Reporting (Auto accident, property damage, injuries to self or others)
This category is for reporting accidents, injuries, damage to UVM property, or near-misses of any kind. These include (but are not limited to): Work-related Injuries of Employees; Injuries of Students and Non-UVM Affiliates, Automobile Accidents Involving a UVM-Owned or Leased Vehicle, Property Damage, and Concerns/Near Misses such as stripped wires, loose bricks, etc. These reports will be handled in confidence but are not anonymous.
The State of Vermont requires employee injuries to be reported to the Department of Labor within 72 hours of the injury.

- A student recieves a minor burn while participating in a lab experiment
- An employee inspecting a confined space receives a head injury
- A water leak in a laboratory damages research equipment

These types of reports go to the Department of Risk Management, which are then processed depending on the type of report and may involve other departments such as Environmental Health & Safety, Police Services, Office of General Council, Human Resources, or others.

Building and Facility Issues (Leaks, electrical, plumbing)
This category is for reporting issues involving building problems, structural concerns, water loss events, electrical problems, and other Facilities-related issues. Facilities Management uses Planon ProCenter to manage requests for both staff and students. The CATSafe App (powered by LiveSafe) can also be used to report Building/Facility Issues by choosing the "Non-Emergency Tip" Option.

What happens after you submit a report?
Reports submitted in Planon are automatically assigned by zone to the appropriate Facilities Management team who will be dispatched to resolve the issue. Reports to SOS will be similarly triaged. Timelines may vary depending on the urgency of the issue. If an issue is safety related, other units such as Environmental Health & Safety or Risk Management may also be notified.
