Common Student Requests

The Dean of Students office at UVM is separate from the individual Dean's offices in your academic college or school. It's likely you should start with your college's Student Services team, but we're here to help clarify and support as best we can.

In order to better support students returning to UVM after taking time away for medical reasons, we ask that a current treating medical provider respond to the questions below. Responses to these questions must appear on letterhead and should be submitted directly to the Office of the Vice Provost and Dean of Students by email to dos@uvm.edu or by fax to 802-656-3467.  For the purposes of this request, “Provider” means licensed healthcare provider (e.g., MD, DO, Psychologist, Licensed Clinical Social Worker, Licensed Clinical Mental Health Counselor, etc.). This document must be received by the university no later than 45 days in advance of the student’s planned return.

Medical Providers, please submit a letter addressing the following prompts:

  • The student’s name (and Student ID number if known).
  • Treatment the student has received since leaving UVM.
  • Your recommendations for on-going treatment and/or support when the student returns to UVM.
  • What, if any, concerns do you have about the student returning to UVM and resuming a full course load.

Health and Safety Resources

Alcohol Edu and Sexual Assault Prevention Online Courses
Body

UVM has been taking active steps to reduce unsafe alcohol use on our campus. The online program AlcoholEdu was established in the summer of 2006 as a tangible population-level prevention program that challenges students' expectations around alcohol while encouraging students to make healthier and safer decisions. For more information, contact the Dean of Student's Office at 802-656-3380. Further information about Everfi, creators of AlcoholEdu, is available on EverFi's Alcohol Edu Website.

If you have been assigned a module as a new student, you will receive an email to your UVM Email account. You can also log-in to see your assigned courses.

All new students are required to take this course. Students will receive a link to their assigned course in their UVM email approximately one month before the start of their first term.

Medical Amnesty Program
Body

In cases of extreme intoxication/incapacitation due to alcohol and/or other drugs, it is imperative that someone calls for medical assistance. UVM's Medical Amnesty Program serves to remove or reduce consequences faced by students in violation of UVM's Code of Student Rights and Responsibilities to achieve the goal of student safety. Amnesty is granted from potential student conduct sanctions for themselves, the person in need of assistance, or the organization hosting the event where the incident occurs.

Center for Health and Wellbeing
Body

The Center for Health and Wellbeing offers a wide range of services to and provides high quality, prevention-oriented student health care, mental health services, a vigorous program of outreach and education, and the Living Well office in the Davis Center.

UVM Care Team
Body

The Care Team's number one priority is to support a healthy and safe community. Occasionally, members of our community find themselves or others in need of additional help and support. If you are concerned about a UVM community member or are concerned about a specific event, we encourage you to submit a CARE form.
