Required Graduation Forms
Each College/School has their own required graduation form. Complete the appropriate form based on the College/School from which you are graduating. If you are earning a degree from more than one College/School, complete the required graduation form for each College/School. Questions about your required graduation form, including your eligibility to graduate, should be directed to Student Services in your College/School.

Larner College of Medicine
Doctor of Medicine (MD) degrees are conferred four times a year in August, October, January, and May. The University celebrates commencement in May of each year. Students whose degrees have been conferred during the academic year in August, October, or January are invited to the next May ceremony. Medical students who plan to graduate this year must submit the Required Graduation Information survey in OASIS during January so the Dean’s Office and Office of Medical Education have all the information needed to recommend your degree to the Advancement Committee and Faculty Senate in order for the University to award your diploma. It is your responsibility to monitor your degree audit in OASIS to ensure you meet the requirements for graduation.

Spring Graduates (May)
- Students who are at full degree completion at the end of the spring semester will have their degree(s) conferred by Faculty Senate, generally the Thursday before the May Commencement Weekend. Review the Faculty Senate calendar for official dates (subject to change).
- Once degrees are conferred, transcripts will be updated within five business days to reflect program completion, graduating class rank, and Latin Honors.
- Diplomas and transcripts will reflect the date of degree conferral, not the date of the ceremony and celebrations.
- Accelerated Master’s Program (AMP) candidates will have their status updated from Undergraduate (UG) to Graduate (GR) within five business days of degree conferral.
- Degrees are conferred by UVM’s President at the University Commencement Ceremony. Diplomas are distributed at the College/School Celebrations. Diplomas include degree information (not specific major information unless the major is part of the degree name), Latin Honors, and Honors College Scholars. Graduates with a financial hold will receive their diploma once the hold is resolved. Diploma Name, as requested by the student on the Intent to Graduate Form, will be used in all other official commencement publications.
- For students who do not attend their College/School celebrations, diplomas are available for pick-up at the Registrar’s Office counter on the 3rd floor of Waterman, the week following May Commencement Weekend. Otherwise, they are mailed within three weeks to the permanent address listed in myUVM; verify accuracy of permanent address. Diplomas not picked up or mailed (successfully) within one year will be shredded. After this period, a replacement diploma order form will be required to print a new diploma.
Summer Graduates (August, September, October)
- Students who are at full degree completion at the official end of summer session will have their degree(s) conferred by Faculty Senate in either August, September, or October. The date is determined by when a student’s degree completion can be verified by their academic unit. Review the Faculty Senate calendar for official dates (subject to change).
- Once degrees are conferred, transcripts will be updated within five business days to reflect program completion.
- Diplomas and transcripts will reflect the date of degree conferral. Diplomas include degree information (not specific major information unless the major is part of the degree name).
- Between course completion and degree conferral, pending degree candidates may continue to receive central communications related to the fall semester. These communications will end once degrees are conferred and records are updated to reflect program completion.
- Accelerated Master’s Program (AMP) candidates will have their status updated from Undergraduate (UG) to Graduate (GR) within five business days of degree conferral.
- Diplomas will be mailed within three weeks of degree conferral to the permanent address listed in myUVM; verify accuracy of permanent address.
