Complete the LCOM Verification Release Form (DOC) and send it to the Registrar's Office by email to LCOMVerify@uvm.edu. 

You can also fax a copy to 802-656-8230, or send by mail to:

There is no charge for this service. Allow two weeks for processing.

If you need fellowship-related ERAS documentation

Request your MSPE and/or electronic official transcript directly from the Registrar's Office. We will email you a PDF that you may upload directly into the ERAS for Fellowships MIDUS system.
