My professor said that I needed a deans note for my absence, how does this work?

If you have a short-term acute illness, which includes a cold (cough, low grade fever, sore throat, runny or stuffy nose, headache, muscle aches), short-lived gastrointestinal symptoms (nausea, vomiting or diarrhea in the absence of abdominal pain), or a positive COVID-19 test, please complete this form so that information concerning your possible need to be out of class may be shared with your college’s dean’s office.   

Students are responsible for notifying their instructors, preferably before missing a class/assignment. Depending on how you are feeling, you may still be able to complete coursework, so discuss with your faculty about what a plan may look like for you to continue academic progress. Instructors have final authority to excuse absences and for coursework extensions. 

It says I am ineligible for services on mywellbeing, what does this mean?

Being ineligible means that you have not paid the health fee. You will need to contact StudentInsurance@uvm.edu and ask to add the health fee to your financial account to be able to be seen at CHWB.  

The health fee goes from January 1st-June 30th and July 1st- December 31st and is $481 per semester. For more information on accessing CAPS and Student Health Services and the Health Fee, click this link for more information. 

I already have insurance; do I need to enroll in the Student Health Insurance Plan?

No, you do not need to enroll in the Student Health Insurance Plan (SHIP) if you have your own, comparable insurance.  

You will need to complete the Insurance Waiver to deny the SHIP insurance and add your personal insurance information.  

We strongly suggest talking to your insurance provider about your specific coverage plan as UVM services are usually considered out of network.  

How am I able to see providers at the Center for Health and Wellbeing?

To be eligible to see a provider at the Center for Health and Wellbeing, you need to pay the health fee. Please visit this link for more information. 

YOU DO NOT NEED TO HAVE THE STUDENT HEALTH INSURANCE PLAN (SHIP) TO ACCESS SERVICES. The Student Health Insurance Plan is different than the Health Fee.  All full time students have already paid the health fee, so please come to our services! 

I want to get tested for STI, Flu, or Strep, can I do this at UVM?

Body

Yes, you can get tested by booking a test and go appointment at UVM Student Health Services. This can be booked online at mywellbeing or by calling SHS at 802-656-3350. Visit this link to see all the various test available.  

Please note that all our point of care tests are not covered by the health fee and will be charged to your UVM account for payment. We do not bill directly to insurance, so to get reimbursed you will need to submit your claim separately using a walk-out statement from your visit.  

How to Print a Walk-Out Statement: 

  • Login to mywellbeing  

  • Select “Account Summary” 

  • Select “View/Print” on the visit you are wanting reimbursement for 

  • Select “Print Statement”  

  • Submit this statement with your claim to your insurance provider  

I see "$XXX" on my student bill, what is this from?

Many things are covered by the UVM Health Fee, but some services like immunizations, point of care testing, and psychiatry visits are not. You can see what is not covered by the Student Health Fee on this page. 

If your visit has a charge, this gets posted to your UVM financial account. To get reimbursed, you need to independently submit a claim with a walkout statement to your insurance provider. We do not bill directly to insurance.  

To get a walkout statement from your visit:  

  • Login to mywellbeing  

  • Select Account Summary  

  • Select “View/Print” on the date of service you are requesting reimbursement 

  • Download this document and submit the claim to your insurance directly 

My insurance rejected/declined my claim for a service at UVM Student Health Services, what do I do?

If your insurance rejects your claim, it is important to follow up and see why the claim was denied. If the walkout statement is missing information, please ask your insurance company to reach out to health@uvm.edu.  

Since UVM Student Health Services is out of network for most insurances, you should check that your coverage covers out of network providers.  

I need to be absent from class due to mental health concerns, surgery, or accident. How does this work?

If you have been seen by an outside provider (ER, hospital, Urgent Care, Primary Care outside of UVM Student Health Services) you should send an email and any documentation from your visit to your dean’s office directly.  

For any mental health concerns, if you are seeing a provider at UVM CAPS, reach out to your counselor through mywellbeing messages and ask for an academic flexibility note to be sent to your Dean’s office.

I have an injury and need parking accommodations, what should I do?

To request a temporary parking permit, you will need to fill out this form with your provider. Once Part 3 is filled out by your medical provider, send the completed form to chwbmif@uvm.edu. For more information, please visit this link

Please note that these parking passes are for 4-6 weeks max. If you have a longer-term injury you will need to apply for a temporary disability permit through the state DMV that your car is registered in. You can find more information here.   

I have ongoing medical concerns that are longer than 6 months or are part of a medical condition I have. Who should I reach out to get support from?

For any ongoing medical needs, that are not due to an injury and will last longer than 6 months, please reach out to the Office of Accessibility access@uvm.edu. They will be able to help with an ongoing accommodation.   

 

If I have a previous diagnosis for ADHD and need prescription refills, what do I need to do?

To get prescriptions filled by a UVM Student Health Provider, you need to submit the ADHD paperwork and be approved prior to making an appointment.  

You must also have a previous diagnosis. We do not diagnose ADHD at UVM. We can give referrals to providers that diagnose in the community if you reach out to health@uvm.edu. More information on what documentation is needed visit this link

Who needs to submit immunization documents and by when?

All new and incoming first year and transfer students are required by the Vermont Department of Health to submit immunization documentation. Please visit this link to view the specific immunizations that are needed.  

 Deadlines for submitting immunization documents: 

  • Fall: July 1st  

  • Spring: December 1st  

How do I submit immunization documentation to UVM?

To submit immunization documentation: 

  • Login to mywellbeing 

  • Select “Medical Clearances” 

  • Enter the administration date for all immunizations listed under “Items Required for Clearence” 

  • Upload one copy of your immunization record (only one copy please) by selecting the option “Immunization Record” under the section “Other” 

  • Please note we cannot verify any of your immunization dates without documentation 

How do I check on the status of my immunization compliance and find out if I still need to submit other documentation?

  • Look at the “Status” of each immunization  

  • If it says “Not Complaint” next to a certain immunization document, we need more documentation or you are missing an immunization

What happens if I am not compliant with immunizations?

If you are not compliant with immunizations a hold will be placed on your account that will prevent you from registering for classes, the following semester.  

Dates Holds are Placed on Student Accounts: 

  • Fall: October 1st  

  • Spring: March 1st  

I want to request a medical withdrawal from school or a specific class; how do I get this process started?

Body

To start the medical withdrawal process you need to make an appointment with your academic advisor to fill out the withdrawal form. Your academic advisor will reach out to CHWB to request confirmation of the withdrawal if it is due to health concerns.  

If you want to start a medical withdrawal but are not connected to anyone in Counseling and Psychiatry (CAPS) or Student Health Services (SHS) you will need to make an appointment with our Behavioral Health Coordinator by calling 802-656-3350.  Please visit this link for more information.  

I want a copy of my medical records; how do I request this?

To get any medical records released from UVM, you need to submit an Authorization for Release of Protected Health Information Form. To access this form:  

  • Login to mywellbeing  

  • Select “Downloadable PDF Forms” 

  • Select “Download” under the section Authorization for Release of Protected Health Information (optional) 

  • Fill out the form completely  

  • Select “Upload” under the section Authorization for Release of Protected Health Information (optional) 

  • Allow 3-5 business days for your request to process

I am having a hard time submitting documents on mywellbeing, is there another way to send documents in?

If you are having a hard time submitting documents on mywellbeing, you can alternatively submit them through UVM file transfer to chwbmif@uvm.edu.

The Center for Health and Wellbeing holds your care in strict confidence.

As professionals, our staff are expected to consult with one another, as necessary, to insure that your care is of high quality.

However, by both law and by the ethical standards embraced by our staff in the counseling and medical professions, we are not permitted to release any information about your care to anyone outside of the Center for Health and Wellbeing without your explicit permission. 

We will not release information about the facts or nature of your care to family, friends, roommates, University officials, faculty, or staff without this permission; a copy of medical or counseling records is available only after we receive a written request signed by you.

There are rare circumstances when we break this seal of confidentiality: when you have a very serious mental or physical health problem and are unable to assume responsibility for notifying others, when your life or that of another is in danger, or when we are presented with a valid court order requiring us to release records.

If you have any questions about how we respect the privacy of your health and counseling information, do not hesitate to ask your provider.

Immunization requirements

All full-time UVM students must have an insurance plan to help cover additional healthcare costs. This includes:

  • Undergraduate/Continuing Education students registered for 9 or more credits
  • Graduate students registered for 9 or more credits
  • Graduate students enrolled in "Grad 903" credits
  • All International Students with F-1 or J-1 visas
  • All Medical Students

Students must complete the insurance decision/waiver each year to declare whether they are enrolling in the UVM Student Health Insurance Plan or if they will waive it for an equivalent insurance plan.

Every relationship with a health care provider includes certain rights and responsibilities. We view your relationship with the Center for Health and Wellbeing (CHWB) as a partnership. We are committed to protecting your rights and providing high-quality, respectful care. In turn, we ask that you understand and meet your responsibilities as a patient.  

Quality care depends on open communication, mutual respect, and active participation. The following rights and responsibilities define our shared commitment to your care.  

Your Rights 

 You have the right: 

  • To be treated with dignity, respect, compassion, and consideration.  

  • To have your cultural background, identities, beliefs, and traditions meaningfully   respected in your care.  

  • To receive services that recognize and respond to you as a whole person with unique experiences and needs.  

  • To receive care in a secure, clean, and accessible environment.  

  • To privacy in conversations, examinations, counseling, testing, and treatment.  

  • To confidentiality of your medical and counseling records, consistent with applicable laws and professional standards.  

  • To consent to or decline involvement of supervised trainees (such as interns or residents) in your care.  

  • To receive information about the scope, availability, and cost of CHWB services, including what is and is not covered by the UVM Health Fee..  

  • When applicable, to review and receive an explanation of your medical bill, regardless of who is responsible for payment.  

  • To designate an authorized individual to receive medical information on your behalf when appropriate.  

  • To express your wishes regarding future care, including designating a health care proxy or completing advance directive documents, consistent with state law.  

Your Responsibilities 

 As a patient, you are responsible for:  

  • Arriving on time for appointments or providing timely notice of cancellation.  

  • Providing accurate and complete information about your current and past health, medications, and concerns.  

  • Informing your clinician promptly of changes in your symptoms, health status, or concerns.  

  • Safeguarding your personal identification and not allowing others to use it for health services.  

  • Actively participating in developing and following your agreed-upon treatment plan.  

  • Sharing relevant beliefs, preferences, and concerns that may affect your care.  

  • Discussing your ability and plans to follow recommended tests, treatments, or health behavior changes.  

  • Using medications as prescribed and not sharing medications with others.  

  • Following up with your clinician as recommended and informing CHWB if you choose to seek care from another provider.  

  • Asking questions if you do not understand your diagnosis, treatment plan, or expectations.  

  • Informing your clinician if you believe you cannot follow the recommended plan of care.  

  • Recognizing that daily health decisions—including use of tobacco, alcohol, or other substances; nutrition; sleep; and physical activity—affect your overall health.  

  • Following instructions for treatment, making recommended follow-up appointments, and using medications or contraceptives as directed.  

  • Understanding the UVM Health Fee and our health insurance coverage, including network participation, referral requirements, reimbursement procedures, and any limitations that may affect payment for services. 

  • Providing accurate and current insurance information and notifying CHWB of any changes in coverage. 

  • Paying the UVM Health Fee and applicable personal insurance fees, co-pays, deductibles, and outstanding balances in a timely manner, including any amounts not covered by insurance or not included in the UVM Health Fee

  • Treating health care professionals, staff, and other patients with courtesy and respect.  

  • Providing feedback to help improve the quality of care and educational services at CHWB.  

 

Treatment of minors (under 18)

Body

State of Vermont law dictates that any individual under the age of 18 is considered a minor and that in many circumstances is not capable of giving informed consent to their own medical care. In simple terms, this means that we need parent/guardian permission before we can treat a student until they turn 18. Once they are 18, permission is no longer needed. There are several important exceptions to this, as noted below.

  • Minors of any age will be cared for in emergency situations which necessitate immediate treatment.
  • Minors 12 years of age and older may give informed consent for treatment for sexually transmitted diseases, drug dependence, and alcohol abuse. If a minor requires hospitalization for treatment of any of these conditions, the parents must be notified of the hospitalization.
  • Minors of any age may provide their own informed consent for the termination of pregnancy, or to receive medical treatment necessary to obtain contraceptive devices and medications.
  • Minors of any age may provide their own informed consent for medical treatment related to rape, incest, or sexual abuse. Health care providers are required to report such incidents to the Department of Children and Families within 24 hours.
  • Minors 14 years of age and older may voluntarily admit themselves to a hospital for mental health care if they provide informed consent in writing.
  • Minors under the age of 14 years may voluntarily admit themselves to a hospital for mental health treatment if they provide their own written informed consent and a written application from a parent or guardian.
  • Minors who are married or have ever been married; minors on active U.S military duty; and minors emancipated by court order may give informed consent to their own health care.
