Club Sports Resources

All members of club sports are REQUIRED to complete the Club Sports Agreement to Participate Form found on the RSO Resource Hub on the Rally Point Homepage. This form should be completed prior to each individual’s first practice with the club. Every member of your Club must complete an Agreement to Participate form  before:

• Tryouts/Practice
• Playing in a game
• Traveling with the Club
• Being granted access to the PEAC course (see below for more info)

Agreement to Participate forms are like a liability waiver, they protect students and you as a Club Leader. Please help keep Club Sports safe by making sure everyone fills one out ahead of participating in Club activities.

Individuals who refuse or do not sign the agreement will not be eligible to participate in Club Sports activities

How to Guide - Agreement to Participate Forms (Scribe)

All active members of club sports are eligible for one PEAC 1889 credit per club sport per academic year. To be eligible to enroll in PEAC 1889, an Agreement to Participate form must be on file (see above) and the PEAC Override Request Form must be completed. To find the form, please navigate to the RSO Resource Hub on the Rally Point Homepage. To obtain credit for a previous year, email Grace Peterson, Assistant Director for Club Sports and Student Organizations (grace.peterson@uvm.edu).

How to Guide - PEAC Credit Request (Scribe)

A Microsoft Form for referees to complete and submit prior to services along with a valid, signed, and dated W-9. If this cannot be submitted ahead of time, it must be submitted within 3 days of completed services.

Completed W-9s should be sent via secure file transfer to sgapayment@uvm.edu: https://filetransfer.uvm.edu/ (you must select “Send to a UVM affiliate”).
