Whole Person Health Index
Developed by the National Center for Complementary and Integrative Health (NCCIH), a vital arm of the National Institutes of Health (NIH), and the Centers for Disease Control (CDC), the Whole Person Health Index features nine (9) questions, is free to use, and can be administered in person or online.
All items use the same response scale:
1 = Poor 2 = Fair 3 = Good 4 = Very Good 5 = Excellent

| Sleep | How would you rate your sleep? |
| Meaning and Purpose | How would you rate your ability to find meaning and purpose in your daily life? |
| Health Management | How would you rate your ability to manage your health, focusing on aspects of your health that matter most to you? |

Reinventing the Wheel: The Whole Person Health Index at UVM
UVM is implementing the Whole Person Health Index, with help from its redesigned Whole Person Health Wheel. This visual tool was developed by Osher Affiliates to support patients, providers, caregivers, educators, students, policy leaders, and communities in addressing needs and optimizing health.
