Developed by the National Center for Complementary and Integrative Health (NCCIH), a vital arm of the National Institutes of Health (NIH), and the Centers for Disease Control (CDC), the Whole Person Health Index features nine (9) questions, is free to use, and can be administered in person or online. 

All items use the same response scale:

1 = Poor     2 = Fair     3 = Good     4 = Very Good     5 = Excellent 

Self-Rated HealthHow would you say your health is in general?
Quality of LifeHow would you rate your quality of life, focusing on what matters most to you?
Social and Family ConnectionsHow would you rate your social and family connections?
DietIn general, how healthy is your overall diet?
Physical ActivityHow would you rate your physical activity, compared with people in your age group?
Ability to Manage StressHow would you rate your ability to manage stress?
SleepHow would you rate your sleep?

The Whole Person Health Index is now implemented in the All of Us Research Program, created by the NIH to accelerate health research and support the development of individualized healthcare, and through cancer centers.View the Whole Person Health Index on the NCCIH website
