  • Care Coordination Collaborative (CCC) meetings bring together interdisciplinary partners to advance our shared vision of optimizing care coordination services for children and youth with special health needs. 
  • Shared Plans of Care Pilot Project. VCHIP and Vermont Information Technology Leaders (VITL) are collaborating with several partners around Vermont to optimize technology to support the utilization and accessibility of care plans for children and youth with special health needs.
