As of May, 2026 the Student Insurance Enrollment/Waiver Form has moved to the Center for Health and Wellbeing web  portal, Mywellbeing.

To enroll in or waive UVM's Student Health Insurance Plan (SHIP), sign on to https://Mywellbeing.uvm.edu with your UVM NetID and select Student Health Insurance Waiver/Enrollment from the menu.
