Getting Started
1. Submit Required Forms
Have your allergist complete the Allergy Clinic Info Checklist and fax it to us:

We must receive this form before you arrive on campus.
SHS Approval Required
Once your documentation is reviewed, we’ll notify you via the MyWellbeing portal if you’re approved for allergy injections or if additional information is needed.
Send or Bring Your Serum and Paperwork
Bringing it with you? Store it as instructed by your allergist and drop it off on move-in day or when you arrive on campus.
Send serum paperwork via secure file transfer to chwb.mif@uvm.edu).
Fax serum paperwork? 802-656-8178

📌 Note: It takes at least 2 business days to review your information. Incomplete paperwork may delay or reschedule your appointment.
Allergy Injection Appointments
What to Expect:
Plan to spend at least 50 minutes per visit
Includes a 30-minute observation period after your injection
We’ll check your health status and confirm any required pre-medications
If you’ve been prescribed an EpiPen, you must bring it to each visit.
If not, we may need to reschedule your appointment
Emergency Protocol:
In the event of a serious allergic reaction:
SHS staff will administer epinephrine
911 will be called, and you will be transported to the UVM Medical Center Emergency Department (UVMMC)

One injection per visit | $25 |
Two or more injections in one visit | $35 |
Charges are billed to your student account
No charge for serum storage during the semester
