Emotional Support Animal (ESA) Requests
The Emotional Support Animal (ESA) request form can be downloaded as its four separate parts:
- Student ESA Request (.pdf) - to be filled out by the student
- ESA Supporting Documentation (.pdf) - to be filled out by the student's mental health professional
- Vet Certification of ESA (.pdf) - to be filled out by the student's veterinarian
