The following is a link to the ASP automated Acccommodations Request Form.

**EBOOK REQUEST FORM**

**Once you have subitted the request on page 3 of 3, you must click the "ebooks requests" menu item to input the format and book information**

 

If you cannot access the eBook request form, you can send an email to eBook@uvm.edu with the following information for each of the text books you would like converted;

Title

Author

Version or Edition number

ISBN (found on back cover near barcode)

or

Class Department (ex. BIO)

Number (ex. 001)

Section (ex. a)

Professor (ex. P. Smith)
