We work in collaboration with our care management team, our integrated behavioral health team and our pharmacy team to achieve improved health outcomes for our patients. Our primary care medical homes are located in Burlington, Essex, South Burlington and Williston.
We are proud to educate medical students, resident physicians and advanced practice providers in our offices and in the classroom setting. We teach evidence-based medicine and emphasize the importance of the longitudinal relationship between patient and provider. We consistently earn high praise from our learners for our efforts.
We are engaged in scholarly activity including quality improvement activities that focus on a variety of topics including: high-value care, health outcomes affecting our population, improving the patient experience, improving the operations of our clinic. Our faculty have earned grant funding to support development of innovative projects and curriculum on topics including: clinical reasoning, exercise for osteoporosis, lifestyle medicine, suicide screening and intervention, anxiety screening and intervention, and cultural humility.
