UVM Cares

Personalized care guidance for UVM employees and their families made possible with the University of Vermont Health Network. Anyone covered on UVM's medical insurance has access to UVM Cares.

Health care is complex—UVM Cares makes it easier

UVM Cares is a dedicated Care Management service designed for UVM employees and their family members enrolled in our medical insurance. It offers personalized care navigation and support, helping individuals engage more with primary and preventative care while also reducing the impact of chronic conditions through broader access to care management.

Through this program, you’ll have access to:

  • A dedicated Care Guide to help answer questions, schedule appointments, and connect you with health resources.
  • Follow-up support after an Emergency Room visit or new diagnosis.;
  • Coordinated care to help manage certain conditions.
  • Confidentiality. Your personal health information remains private, and the university does not have access to your medical data.
