Access to Facilities
1. Completion of Training Modules
Research personnel must complete the general lab animal training and the species specific course(s) for each species listed on their protocol. Detailed instructions and a place to check for completion is at UVM Research Protections Office (RPO) — CITI Program Training.

2. Submission of Animal Handler Occupational Health Questionnaire to IACUC
The Institutional Animal Care and Use Committee (IACUC) requires submission of an Animal Handler Occupational Health Questionnaire (PDF). This form can be found on CITI Training - IACUC page under the heading "Occupational Health and Safety Program."
NOTE: The Animal Handler Occupational Health Questionnaire must be completed every three (3) years.
3. Inclusion on Protocol Roster for Relevant IACUC Protocol(s)
Principal Investigators (PIs) must submit a Protocol Amendment to the Institutional Animal Care and Use Committee (IACUC) to add new personnel to protocols. No one is given access to the Animal Facility without first being added to a protocol roster.
4. Hands-on Biomethods Training
This training is required before access to facilities can be granted. Biomethods training can be scheduled and completed once personnel has been cleared by Occupational Health to handle animals and the amendment to add them to an approved IACUC protocol is either in-progress or approved. Facility access will not be granted until personnel are added to an IACUC protocol in CLICK. Contact the OACM Veterinary Technicians for their schedule at VetTech@med.uvm.edu or call (802) 656-8708. 5. Submission of Key/Catcard Access Request Form to Facility Manager
After completion of the above requirements, fill out a Request for Keys/CATCard Access Form (PDF) found at the Physical Plant Department — Lock Shop. The specific information for our facilites is provided below.
For access to:
- Given animal facility, write "Given Animal Care"
- HSRF animal facility, write "HSRF Animal Care-001"
- HSRF 005A, write "HSRF Animal Care-005A"
- CRF animal facility, write "CRF Facility Animal Care"
- 4th floor of Dewey, write "Dewey 4th floor Animal Care"
- Stafford animal facility, write "Stafford Animal Care"
Note: Access is granted for 2 years after which you will need to re-submit an Access Request Form (This does not apply to Principal Investigators [PIs]). If you are not a PI, please request Temporary Access. You do not need to supply a Key Return/Deactivation Date, the Lock Shop will automatically select a 2-year end date for this request.
Email the completed form to the OACM Facility Manager at: FacMgr@med.uvm.edu or submit form to a Vet Tech during your biomethod training.
The OACM Facility Manager will review the information before passing it on to the Lock Shop. It can take up to 5 additional days for your request to be completed.
