Academic Planning Forms - Pre-Decision Phase of GoAbroad
Choose the worksheet for your academic school or college to use in your pre-application advising with your academic advisor. You will attest to having completed this requirement when you apply in GoAbroad. Follow any directions on the worksheet detailing where you should turn it in.

Study Abroad Approval Forms - Post-Decision Phase of GoAbroad
Students are required to meet with their academic advisor in the post-decision phase of GoAbroad to discuss the types of classes the student plans to take while abroad and receive academic approval to go abroad.

Study Abroad Academic Eligibility Exception Form
Occasionally, students may not meet all of the eligibility requirements to study abroad but should be considered for exceptional circumstances. This is facilitated by our Student Abroad Eligibility Exception Form.

Budget Sheet Template
Budget sheets assist UVM Student Financial Services in adjusting relevant aid and help students better understand the costs of their programs.
If you are participating in an Exchange, ISEP Direct, UVM Semester, or Direct-Enroll program, OIE will prepare your budget sheet for SFS. SFS will email you with next steps. You do not need to complete this form.
If you are participating in an approved external program, you will make your own budget sheet using the template below. If you are completing two consecutive semesters on two different study abroad programs, you will need to make two different budget sheets.
Medical Clearance Forms
All students participating in Travel Study/UVM Field Studies programs complete a Medical Clearance Form. These are customized to the program - select the correct form for your program.
