Scheduling starts with a quick 5–10-minute phone call to collect your date of birth, address, the purpose of the appointment, and basic insurance information. Then we will send you a packet of paperwork. Once you return the paperwork, we can schedule your first appointment.


The following forms will be included in your packet:

  • case history 
  • registration 
  • request for report distribution
  • permission to correspond by email
  • HIPAA
  • training of students consent
  • tele-practice
  • driving directions
  • services and costs
