Policy for Mandatory Health Insurance

Insurance Policy Waiver/ Decision Form

Dear UVM Student:

Health Insurance is mandatory at the University of Vermont.


Each year you need to complete a form to either:

  • purchase the University of Vermont Sponsored Health Insurance or,
  • decline/waive participation in the sponsored health insurance by identifying comparable coverage which is currently in force with another insurance company.

University of Vermont Policy for Mandatory Health Insurance:
All undergraduate, Continuing Education and Certificate students enrolled in at least 9 credit hours, graduate students enrolled in 12 or more credits, or Henderson Pre-doctoral Fellows are required to have health insurance that provides equal or better coverage than the University of Vermont Student Insurance Plan. The UVM per semester Health Fee which covers on campus services is a prerequisite to purchasing the Student Health Insurance. This service along with the Student Health Insurance, provide a comprehensive health care plan.

All full and part-time international and exchange students are required to pay the UVM Health Fee and maintain adequate health insurance coverage. This requirement ensures the student’s access to adequate health care and protects against potential financial liabilities, which might cut short educational plans and create financial hardship in the future.

Undergraduate, Continuing Education and Certificate students taking from 6 to 8 credits and all other students, including graduate students, taking from 6 to 11 credit hours are eligible, but not required, to purchase the Student Health Insurance Plan and pay the prerequisite per semester Student Health Fee. Graduate students desiring coverage, but enrolled in fewer that 6 credits should contact the Graduate College for further information.

The Health Fee, which is part of the comprehensive fee, billed to 9 credit hours for undergraduate, Continuing Education and Certificate students and for graduate students with 12 or more credit hours, supports access to appointments at NO additional charge in the: Student Health Medical Clinic, Women’s Health Clinic and Counseling Center as well as services from the Office of Conflict Resolution, Alcohol and Drug Services, Mindfulness Meditation, Nutrition Counseling, Health Promotion, and Athletic Training for Varsity Athletes.

Henderson Pre-doctoral Fellowship Policy for Mandatory Health Insurance: UVM is very committed to supporting the Henderson Pre-doctoral Fellows and as a result we require that they have health insurance. Although not technically a graduate student at UVM, Henderson Pre-doctoral Fellows are mandated either to sign up for the same health benefits as the UVM Graduate Students or need to provide proof of insurance coverage from their home institutions.

Insurance Decision Waiver Process
Because insurance is mandatory, you must fill out an insurance decision/waiver form online that either:

  1. purchases the University of Vermont sponsored Student Accident and Sickness Insurance or
  2. declines participation in the insurance by identifying comparable coverage currently in place with another insurance company.


This waiver has to be done on a yearly basis regardless of your insurance decision. Students who do not fill out a waiver by the deadline will be billed, the insurance premium will be added to their student account, and have a hold placed on registration.


We have an on-line waiver process for all students, except international students, that will help you meet your insurance requirements.
International Students: Please Note: You will not be able to use the on-line waiver process. Insurance and the health fee is mandatory for all international students and you will automatically be billed. If you have any questions about this requirement, please contact the insurance office at 802-656-0602.

Instructions for all other students
• You must complete the Insurance Policy Waiver/Decision Form online to make your insurance selection by the deadline. Insurance decision/waiver forms will not be accepted after September 30, 2007 for the Fall semester and February 15, 2008 for the Spring semester.

• If you elect to waive you are legally responsible for any medical expenses incurred during your enrollment at the University. The University and its medical insurance program will not be responsible for any of your medical expenses.

• The University of Vermont and Chickering Claims Administrators Inc. reserve the right to verify insurance information. If your plan does not provide comparable coverage to the Student Insurance Plan, or you are uninsured, you will automatically be billed and enrolled into the Student Health Insurance Plan and have a hold placed on registration.

• Please be sure that your plan meets the following criteria before filling out the waiver.

Some Important Facts to Consider...

Please consider these facts about Health Insurance for college students. Yes or No?

1. Will you reach your age limit on your parent’s policy this year?
2. Does your insurance cover you in the Burlington, VT area and the Center for Health and Wellbeing on UVM campus for non-emergency care?
3. Will your insurance cover you, if you are traveling abroad?
4. Does your insurance cover prescription drugs in the Burlington, Vermont area?
5. Does your plan have at least $50,000 in coverage?

Students who are taking at least six credits hours (Graduate students included) are eligible, but not required, to purchase the Student Health Insurance Plan or pay the prerequisite student health fee. A brochure is available on-line at the Chickering-UVM Customized Student Insurance Web Page for you to look at or print out (select the "Policy Info" link to access the plan brochure). Why Purchase A Student Health Insurance Plan?

  • Enrollment periods are specific to your academic schedule.
  • Enrollment requirements do not depend on age, but on student eligibility status as determined by your school.
  • Coverage is continuous during your academic career, 24 hours a day, anywhere in the world--summer break included.
  • The cost of a student health insurance plan is typically only about one-third the cost of an employer plan or individual coverage.
  • The student health insurance plan coordinates care with each campus and its local community providers to ensure access to the best medical care.
  • In most cases, a student health insurance plan will include a worldwide travel assistance service offering unlimited emergency medical evacuation services.
  • Spouse and dependent coverage is available at most schools.

Supplemental Medical Coverage
The aggregate maximum benefit under the Student Health Insurance Plan described above is $50,000. If you purchase the Basic Student Health Insurance Plan at the University of Vermont, you are eligible to purchase the Supplemental Plan of $200,000 at a small additional premium. This would extend the combined maximum to $250,000. You should give serious consideration to purchasing this enhanced supplemental coverage.

Supplemental Plan Enrollment Forms and pricing are available by contacting the Chickering Group at 1-800-286-1144 or on-line at the Chickering Group Web site to view the supplemental Enrollment form (select "Policy Info" to access the Supplemental Application form). Print and complete the Supplemental Form and Mail it to the Chickering Group along with your payment.

Spouse/Dependents/Same Sex Spousal Equivalents
You must enroll with Chickering directly by calling Chickering at 1-800-286-1144 or on-line at the Chickering Group Web site (the policy brochure and Dependent Application form is available by clicking "Policy Info").

If you choose to waive the University’s insurance, keep in mind that there may be services provided (e.g. lab tests, x-rays, referrals etc.) that might not be covered by your insurance company. You will be responsible for paying such charges. It is your responsibility to learn about your health insurance so you know what is covered.
Please note: Before using the on-line waiver decision form students will need to have their Student ID# and insurance information. New students can begin this process after they have registered for classes.  Not sure what your Student ID# is?  It's the 9-digit number you use to register for courses.
