Forms and Submission Requirements

NOTE: You must submit all listed forms electronically to iacuc@uvm.edu as of 10/01/09.

We require scanned PDF format with appropriate signatures.

Initial Submission Guidance
  • Animal Use Protocol Form (revised 11/20/12 NOTE: It is important that you provide a protocol version date on the footer of your protocol.)
  • Request to Add a Commercial Farm for Purposes of Research or Teaching (updated 09/21/12 - Submit this supplemental form when you wish to conduct research at a commercial farm. )
  • Personnel Roster (06/27/11 - The roster is required ony at time of initial submission.)
    • All Pain Levels
      • submit PDF version of form with vet, scientific review and PI signatures
      • submit PDF version of any other supporting materials (A grant is no longer required if protocol form has an Infoed Proposal Number included.)
    • Protocols Identical to Previously Approved Protocols
        • submit PDF version of form with vet, scientific review and PI signatures
        • submit PDF version of a signed memorandum The memorandum should explain that the protocol is identical to a previously approved protocol with regards to hypothesis, specific aims, and animal species and use. Reference the approved IACUC file number and protocol title in this memorandum.

 

 

Subsequent Submissions Guidance
  • Amendment Algorithm
    • Request for Change in Principal Investigator or Key Personnel (updated 09/21/12) (If you have listed more than one person on your form, please make sure they have all completed their required training and submitted their occupational health form.)
      • submit PDF version of form with PI signature (05/02/11 - Note: There is no need to attach a separate Personnel Roster form.)
     
    Continuing Review Guidance

    Beginning with the May notices we will be forwarding to the investigators a reminder notice but no form. The notice contains a web link to our forms page so that you can obtain a Word version of the continuing review form. Within this reminder we will notify you if it is a regular or triennial continuing review and provide you with the numbers of animals, approved and used, to allow you to fill out Section 4 of the form appropriately. Make sure that you reference the notice when you are ready to complete the form. You will also receive a key personnel roster within the notice. The roster needs to be printed, completed and attached to the completed continuing review form for submission. If you have any questions about this new process, contact our office.
