How to Obtain I-Team Services:
Request for Technical Assistance Process: 2012-2013
To request technical assistance (TA) with I-Team please refer to the appropriate TA Request Form (below) as there is a form for children ages Birth-5 and a separate form for those ages 6-22. Once completed the LEA signs authorization and has the form submitted via hard copy to our physical address or to our confidential fax. Please note that both LEA and parent/guardian written permission are needed to begin technical assistance with the school team.
Technical assistance generally includes at least one on-site visit, contact to the family by the Family Resource Consultant to explain I-Team services and to assist in answering questions the family may have, a meeting with the case manager/school team to review the process and discuss the team’s request for technical assistance, an observation of the student within the educational setting, a follow-up meeting to provide recommendations based on observations and the team’s concerns, and follow-up with the school team/family on recommendations.
Because IEP team membership, and the student/family needs change annually, a request for technical assistance form must be completed for each student each school year. Remember that both the LEA and parent/guardian signature are necessary. The 2012-2013 TA Request Forms are available for download below.
Please ensure that the student's family is given an opportunity to provide input on the TA Request Form. The IEP team is responsible for keeping the family informed of all I-Team visits. Family members are always welcome and encouraged to be present when I-Team members visit the school.
Once you are sure that all the information is complete, please make a photocopy of the completed TA Request Form for the student's file. Then mail the original referral form to Louise Lynch at our main office in Burlington. You may also FAX the form to the confidential I-Team FAX line at 802-656-3636. Louise will record the referral and forward it to the I-Team Director, Regional Educational Consultant and Family Resource Consultant.
