Scheduling an appointment is easy.  When you call our scheduler, you will be walked through an intake process.  We need preliminary information such as your date of birth, address, the purpose of the appointment, and basic insurance information, and then we can provide you with an appointment date and time. We will then mail you a packet of paperwork that includes the following:

  • a history form
  • an insurance form
  • consent for treatment form which includes your acknowledgment that you will be working with students under the supervision of an audiologist or speech-language pathologist
  • general fees information

We ask that you complete the paperwork and mail it back to us in the self-addressed, stamped envelope we provide at least two weeks prior to your appointment.  If you have any questions about this process, please ask the scheduler when you call.

Our thoughtful and professional staff will make each of your visits to our clinic pleasant and comfortable. Our staff is an integral part of our clinical team and they work hard to keep the office running smoothly. They make sure individuals who come to our clinic feel cared for and respected.
