Printing Instructions

This form was designed to fit on 8½" x 11" paper with a 1" margin on the top and bottom and with ½" margin on the left and right.

Ordering options:

1. Call Toll free 800-331-7305 or 802-656-3290 to place an order by credit card. Please have your card number and expiration date available. No C.O.D orders accepted.
2. Print and mail this Catalog Order Form with payment. Make checks payable to: University of Vermont. Credit card users please complete the credit card information section, sign and mail order to: The University Store, University of Vermont, Burlington, VT 05401.
3. Print and Fax this Catalog Order Form including credit card information and signature to 802-656-8317.
4. E-mail order form including credit card information to University.Store@uvm.edu. This information will be forwarded to our mail order department. Please note: This is NOT a secure transaction. However, we do not give away or sell this information. We will add your name and address to our mailing list only. We do not retain any credit card information.

We will replace or refund any items exclusive of shipping & handling. Not responsible for typographical errors. No coupons or discounts apply and mail orders are not included in Store sales.
