Medical Home
The Medical Home clinical focus area includes:
- Family-Centered Care & Cultural Competence Assessment & Improvement Plan: Each fellow will complete an assessment of a Medical Home practice focused on family-centered care and cultural competence.
- Participation in Community Consultation & Tools Training: Fellows will be part of the interdisciplinary team completing one community-based assessment. Tools and strategies for use in the assessment process will be taught and the fellow will participate in the assessment in the community.
- Care Coordination
- Enhanced Technical Assistance: Technical assistance from our interdisciplinary faculty to support specific clinical issues.
