VT-ILEHP COMMUNITY ASSESSMENT PROCESS

Community-Based Assessment:

We provide assessment for children with special health and developmental needs in their home, school, and/or other community environments where the family and providers feel we will have the best opportunity to see the child in his/her natural situations. A trainee or fellow is assigned as the assessment coordinator.

  • Referrals are made by family members, health care providers, schools, and other agencies.
  • A community based intake interview is done to gather the concerns and questions of the family and team.
  • Pre-assessment planning occurs with the full interdisciplinary team in collaboration with family and team.
  • Community visits are made with an interdisciplinary team of 4 to 6 disciplines participating directly in the assessment.
  • Assessment information is integrated to address identified questions and presented to the full interdisciplinary team, family, and community team who collaboratively develop recommendations.
  • An interdisciplinary report is developed and a resource notebook is created.
  • Follow-up meetings are scheduled with the family and community providers to create action plans for implementation of recommendations.
  • Assessment coordination is available following the VT-ILEHP community-based assessment to assist with implementation of recommendations.
  • The assessment coordinator is the liaison between VT-ILEHP and the family and their team for the remainder of the academic year.
