Target Areas of Transition
Living Arrangements

Supports provided to assist a person to live in the community.  Supports can be within the family home or in a variety of settings outside of the family home such as apartments or group homes with varying levels of support.

o 2. Outside the family home with support (within five years of graduation).

Employment

Supports provided to assist a person to participate in employment in an integrated setting.
(Check one only)

o 1. Short-term employment supports (career counseling, job seeking, limited training, etc.).

o 2. Supported employment services (assessment, job development, on-site training and supervision, and  follow along).

o 3. Intensive supported employment services (one-to-one support on the job for more than 12 months).

Post-Secondary/Continuing Education

Assistance required to enable a student to apply for, enter and/or maintain participation in any post-secondary college, university, trade school, vocational education program, or other continuing education.

Community Participation

Assistance in developing or coordinating appropriate supports, services and skills for the individual to fully participate in community activities.

o 1.  Service coordination (assistance in assessing an individualís needs and identifying and coordinating appropriate supports/programs in the community).

o 2.  Individual skills (independent living skills, self-advocacy skills, transportation skills, self-care skills, communication skills, recreational skills, etc.).

o 3.  Support personnel (attendant care, readers, drivers, etc.).
