LIFE AFTER HIGH SCHOOL
Transition Planning

Dear Student, Family Members and Friends,

This year at your IEP meeting (Individualized Education Program) you will be
asked about your plans and goals for the years after you leave high school.

You will be asked about such things as:

   * What kind of job or career do you want?
   * Do you want to go to college or vocational training?
   * Do you want to live with your family or do you want to live elsewhere
     or on your own?
   * What kind of community activities do you plan to participate in?

This form was given to you for four reasons:

  1. To assist you as you think about your future goals and plans.
  2. To assist you in identifying things you want to do.
  3. To guide you in planning activities to achieve your future goals.
  4. To help us match your chosen goals with a plan we can all work together
     on to support you in reaching these goals.

EMPLOYMENT

Here are some possibilities you can think about ...
