FORMS
The following forms can be downloaded using Adobe Acrobat.
Quotation Evaluation Form - This form must be completed for all purchases greater than $25,000, but less than $50,000.
Non-Competitive Bid Statement - This form must be completed for all purchases greater than $25,000 when a waiver from the formal solicitation of bids or proposals is requested.
Independent Contractor vs. Employee Status Determination Form - This form must be completed for all purchases of services from individuals (using a social security number to report taxes).
Vendor W-9 Form - This form must be on file with Procurement for all vendors paid by UVM
