Campus Planning Services Forms

Project Request Form -  A department or office seeking changes in its facilities should approach the appropriate dean or director charged with reviewing such requests.  Complete a form for Projects that might include:

  • Renovating or expanding existing facilities.
  • Relocating to other facilities
  • Constructing new facilities

Space Request Form* – Space Request forms must be completed and submitted to Campus Planning Services when unit to unit transfers of space are being requested or when Space is being requested by a unit, with no space yet identified to fill the space need.  Please review the Space Management University Operating Procedure prior to submitting space requests.


Space Bank Form* – Complete the Space Bank form to identify space that your Unit is willing to vacate if and when another unit has a high priority need for additional space.  Space listed in the Space Bank remains the responsibility of the assigned unit until that time when a space transfer is processed with the Provost’s Office approval. 


Space listed in the space bank is identified in the Space Bank Report.
*Note:  The Provost’s Office pays close attention to requests from Cost Centers that want to acquire space.  Cost Centers will have to demonstrate a strategic need or resulting cost savings.

Mapping & Facilities Data Request Form - Campus Planning Services can provide mapping and facilities data. Please complete and submit the form to Campus Planning Services to complete your request.

Note: These forms need to be filled out and saved, and then emailed back to the contact on the form or to plan@uvm.edu.
