UVM Laboratory Safety Notebook
In order to comply with government
regulations and good laboratory
safety practices and facilitate
laboratory safety audits, every
UVM laboratory is required to maintain
a Laboratory Safety Notebook. All laboratories must be
sure to include items 1 through
6 below. The other sections are
designed for model laboratories.

2. Chemical Use Planning Forms for
lab process planning and approvals
of IACUC protocols
3. Location of Safety Information
Note: for the information items, either hard copy and/or bookmarked on an available computer is acceptable.

5. Hazardous Chemicals of Concern Inventory
The UVM Laboratory Inventory, formerly known as the HCOC survey,
is now completed on line at http://www.uvm.edu/~esf/labinv.html. Use the "Show information for this laboratory" button to print your information and store it in the laboratory notebook for easy reference in case of emergency.

Place your Laboratory Safety Notebook on the back side of the main entry door to the laboratory (or an adjacent wall). If the notebook cannot be placed in this location, please post a note there which clearly states where the notebook can be found.
