Schedule an Appointment

Since we are a teaching facility, scheduling an appointment is a little different than the process with which you may be familiar. When you call our scheduler, you will be walked through an intake process where we gather preliminary information (such as your date of birth, address, the purpose of the appointment, and basic insurance information). We will then mail you a packet of paperwork that includes a history form, an insurance form, consent for treatment form which includes your acknowledgment that you will be working with students under the supervision of an audiologist or speech-language pathologist, and general fees information. Once you've filled out the paperwork and mailed it back to us in the self-addressed, stamped envelope we provide, you will be contacted to schedule an appointment. If you have any questions about this process, please ask the scheduler.
