Staff Performance Appraisals

College policy mandates the completion of an accurate performance appraisal form for each staff member in the department at least once a year. The purpose of this exercise is to evaluate the employee's strengths and weaknesses and to give the supervisor and employee an opportunity to exchange job related information. For these reasons, the evaluator must meet with the employee to discuss the appraisal.

Please use one of the following performance appraisal form options:

The evaluation should be conducted by the staff member's immediate supervisor (who would then be evaluated by the Chair) or the Department Chair. Annual evaluations for each staff member should be completed during the spring semester. All evaluations should be submitted to the Dean's Office by the end of April of each year.

In cases where employment and/or performance problems persist, the supervisor or Chair is expected to implement the Constructive Discipline Process. The primary goal of this process is to develop and retain efficient, productive employees.
