How to Request Services
Routine Maintenance & Service Requests
Staff and faculty should submit routine maintenance requests for office, departmental or building concerns through the FAMIS system. Routine requests include situations such as: broken doors or windows, leaky faucets, malfuntioning lights, and other electrical, plumbing or mechanical issues.
FAMIS is the on-line facilities management application that is used for tracking all maintenance, repairs and operations within our department. Using FAMIS is the most efficient way for you to report problems and to provide the detail that we need to process your request.
Students living on-campus should submit concerns about their room and residence halls to the FIX IT program of Residential Life
Please review the UVM Key Policy and the UVM Key & Electronic Access Procedure on the Lock Shop website prior to submitting a request for keys or lock changes. Then, complete the Request for Keys/Card Access Form and fax a signed copy to the Lock Shop directly (Fax Number 656-8537). For more information, phone the Lock Shop directly at 656-0984.
Surplus Property Pickups & Recycling Requests
Cleaning out an office? Submit a FAMIS request to obtain
carts for extra pickup of trash or recycling from your office,
or to request pickup of surplus furniture, computers or other bulky waste (e.g.
wood pallets.) For pickup of appliances,
laboratory equipment and moveable equipment, a supplemental
form is required in
addition to the
on-line FAMIS request. Departments must download
and complete this Surplus Disposal Form
for all appliances or laboratory
equipment regardless of value. (This form is not needed for furniture
and computer pickups) Complete form, sign it, make a copy. Attach one copy to
the actual item to be picked up; and send original copy to UVM Recycling
via campus mail or email. For more information on this

Who pays for what?
Most routine repairs and maintenance to University general-fund buildings and facilities are covered by the Physical Plant's operating budget. However, some services and special requests are charged directly to the requesting department.
Contact SOS to determine if the requested work is your department’s responsibility or covered by central funds. (If requested, Physical Plant will provide an estimate on the cost of a project (e.g., building bookshelves or installing an outlet.) For an estimate, submit a FAMIS request, and indicate "for estimate only.")
Staff and faculty may log in to FAMIS Single Sign-On using their UVM NetId and Password.
NOTE: If you receive a "Secure Connection Failed" error message, then please check which web browser you are using. We've recently learned that versions of Firefox have a security setting that prevents connections to the FAMIS application server. Please only use Microsoft Internet Explorer to access FAMIS for submitting Self Service Requests. Apple/Mac users can continue to use Safari browser to access FAMIS.
Once you're logged in, click on the "Self Services" tab to enter your Service Request.
