Fill in the following fields: Req Type (OL for on-line), Account No., Pct, and Contact. Fields that should be filled in where possible are Start Date, End Date (if the order is a blanket, maintenance, or lease) and Quote Date, Quote Ref (to tell us where the pricing was obtained).
