Bias Incidents

Form » Bias, Discrimination, & Harassment Incident Reporting Form
If you have been the victim of a bias incident, or you know someone who has, please fill out the above form. Descriptions of bias, discrimination, and harassment are found on this form.

Police Services works in concert with the Office of Affirmative Action & Equal Opportunity and the Dean of Students in alignment with our institutional commitment and agency mission to eradicate oppression, bias, and intolerance at the University of Vermont.

For more information, see the Dean of Students' Bias Response Program. If you are unsure of whether or not to submit an incident, please do not hesitate to make a report. You can also contact the Office of the Dean of Students at 802-656-3380 if you are comfortable with speaking to someone, and they will be eager to support you.

When UVM receives a bias incident report, trained investigators with the UVM Office of Affirmative Action and Equal Opportunity (AAEO) assess whether the report involves criminal activity and/or a violation of UVM policy. AAEO will refer bias incident reports to police in appropriate cases. If a suspect related to a bias incident report is identified by UVM Police, and has reportedly violated campus policy or any laws, the incident may be addressed through the campus judicial process and/or the criminal process.
