The IRB has developed this module to help researchers understand the process of informed consent and the requirements for acquiring a fully-executed informed consent document.

Below are pictures of what the signature page of an informed consent document must look like for all IRB approved informed consent documents:

Informed Consent Policies

Open the Research Manual to specific sections or policies:

• SECTION: Informed Consent

• SECTION: Waiving Consent (Partial, Full and/or Documentation)

Create an Informed Consent Document

Remember to keep a "clean", electronic, version of your consent document. This "clean" copy must be resubmitted at Continuing Review, and will be required for making any changes to the document.
