Approved Consent Forms
The IRB places an approval stamp on the signature page of every consent document that is approved for use in humans at our institution. The stamp contains an expiration date, beyond which the consent document can no longer be used.
The approved version of the consent document should be copied for enrollment purposes. A master copy of the consent document should be kept separate for future use - for copies and for administrative reference. Every copy of the consent document presented to participants must have a non-expired approval stamp on the signature page. See what the stamp looks like under each of the below categories:
