If you received your MD from UVM and are looking for verification documentation:

Please complete the File LCOM Verification Release Form (DOC) and send it to the Registrar's Office by email to LCOMVerify@uvm.edu. 

You can also fax a copy to 802-656-8230, or send by mail to Medical Education Verifications, Office of the Registrar, University of Vermont, 360 Waterman Building, 85 South Prospect St., Burlington, VT 05405.

There is no charge for this service. Please allow two weeks for processing.

If you need verification of internship, residency and/or fellowship:

Please review information provided by the Office of Graduate Medical Education at the University of Vermont Medical Center.

Please note: The Registrar's Office does not verify laboratory assistant training or research. Please contact the specific department directly.

If you need fellowship-related ERAS documentation:

Request your MSPE and/or electronic official transcript directly from the Registrar's Office. We will email you a PDF that you may upload directly into the ERAS for Fellowships MIDUS system.
