Housing & Meal Plan Contract
Each incoming first year student is required to complete a Housing & Meal Plan Contract by May 1st. This document is legally binding for the nine-month academic year and clearly outlines a student’s responsibilities as a member of the UVM residential community (including Residential Life’s policies and procedures). Students will be notified of their housing assignments in mid-August.
Health History/Insurance Decision (Due July 1st): These forms will become the basis of your confidential electronic health record while you're at UVM, so make sure you provide us with information that is accurate and complete. Proof of health insurance is mandatory at UVM. Students must complete an insurance waiver/decision form online each year to declare current insurance information or to purchase the UVM Student Health Insurance. These online forms are due by July 1st to the Center for Health & Wellbeing.
Immunization (Due July 15th): Students must submit hard copy documentation of immunization history and also enter the information online. Acceptable evidence of a student's immunization history includes official school, physician or public health records, or laboratory report of a titer indicating immunity to a specific disease. Immunizations should be obtained from your current health care provider and documentation must be sent to UVM's Center for Health & Wellbeing.
