Here’s what to expect after requesting services from the I-Team Early Intervention Project:

The Point Person

When you submit a request for services from the Early Intervention team, we assign you a Point Person. This is a member of our team who will be your primary contact and will facilitate the intake meeting. You should hear from your Point Person within one calendar week of submitting a request for services.

The Point Person will share a link to our provider intake survey, for all team members and the family to fill out. This will help us better understand individual provider’s concerns and needs and will streamline our intake process.

The Intake Meeting

In order to begin working with you, and your team, and your child, we will set up an initial intake meeting. This meeting will include all four of the I-Team Early Intervention team members. We require that the service coordinator and parent(s)/guardian(s) be present. And we request that as many local providers attend as possible.

Your Point Person will email the Service Coordinator options for scheduling the intake.

Before the Intake Meeting

Before the intake meeting, we request that the Service Coordinator to send us the following:

  • The current One Plan
  • Recent provider evaluation reports
  • All pertinent medical/specialist reports
  • A brief video clip of the child, demonstrating your central questions

At the Intake Meeting

The intake meeting will be facilitated by your Early Intervention Point Person. We'll use a routines-based format, as that gives us a full picture of the family’s day and allows us to determine team priorities.

Intake Meetings are scheduled for 90 minutes and are best as stand-alone meetings without other agenda items.

After the Intake Meeting

After the intake meeting, your Point Person will send a Consultation Agreement to the full team. This document will list team priorities, anticipated outcomes of I-Team Early Intervention involvement, and the initial action plan.

The I-Team Early Intervention consultants will get in touch with providers to plan co-visits and provide additional and ongoing resources.
