Supervisors or Dean's Office staff should use this form to request new or updated access to PeopleSoft HR or Financials for an employee. This form will generate a new tracking ticket in the PeopleSoft HelpDesk Footprints system, route it to the appropriate team for resolution, and automatically generate emails to you and other assignees as the issue is updated and ultimately resolved/closed.

Access for terminating employees is automatically removed on their termination date. You do not need to fill out this form for terminating employees.
