Who is eligible for VSAP services?

Any child or young adult, birth through 22 years of age, who has a combined vision and hearing loss, including those with additional disabilities is eligible for VSAP Services.

How do I request VSAP services?

Eligibility Forms may be completed by families, school personnel, related service providers, medical professionals or additional personnel providing services to children and youth with combined vision and hearing loss. If you are interested in receiving technical assistance and/or training for your child or student, please complete and return the Eligibility and Parent Permission Forms to the Vermont Sensory Access Project to ensure that the child or young adult is enrolled in the Vermont Census. Once this information is received, you will be contacted to determine what the specific need is and the best time to schedule the visit.
