Captioning Request Form: General

List below all titles of films or streaming/online video URLs to be used for non course related purposes.

Please make arrangements to drop off and pick up the media at the Universal Design Tech Lab (UDTL). We are located in Room 200 in the Bailey Howe Library.

Please remember to allow 2 weeks for captioning videos/media over 10 minutes and 3-4 business days for short videos less than 10 minutes. Although we strive to satisfy all requests, we cannot guarantee captioning outside these timelines.
