New employees are required to be vaccinated and encouraged to be boosted against COVID-19.

Individuals can make an appointment for their first, second or booster dose(s) through a local pharmacy or through their primary health care provider. The Vermont Department of Health also maintains a list of vaccination sites on their website.


Questions & Answers About Submitting COVID-19 Vaccination Information

Instructions for Submitting Proof of COVID-19 Vaccination

Employees were required to have and submit documentation of their final COVID-19 vaccine dose via PeopleSoft by January 4, 2022.  New employees are also required to submit proof of vaccination.  Acceptable proof of vaccination status includes:

  1. The record of immunization from a healthcare provider or pharmacy;
  2. A copy of the COVID-19 Vaccination Record Card;
  3. A copy of medical records documenting the vaccination;
  4. A copy of immunization records from a public health, state, or tribal immunization information system; or
  5. A copy of any other official documentation that contains the type of vaccine administered, date(s) of administration, and the name of the healthcare professional(s) or clinic site(s) administering the vaccine(s).

Employees may upload their vaccination information, including an image of their vaccine card, through the PeopleSoft HR portal.  The card must clearly include the employee’s name, dates of vaccination, type (brand) of vaccination, and vaccine lot numbers.  See the step-by-step instructions for more details on uploading your vaccine documentation.

*Vaccination cards will be treated as medical information, and will be appropriately safeguarded and kept separately from other employee records.

Vaccine Exemption Requests

Employees may request an exemption for either medical or religious reasons by attaching the applicable exemption request form to PeopleSoft vaccine submittal. See the STEP-BY-STEP INSTRUCTIONS for more details on uploading your vaccine documentation.

  • Medical Exemption Form – Employees may request a medical exemption if receiving a vaccine would jeopardize their health.  The licensed health care provider attesting to the employee’s need for a medical exemption must be someone who is able to administer a vaccination (e.g., not a chiropractor or optometrist). 
  • Religious Exemption Form – Employees may request a religious exemption if their sincerely held religious beliefs (not philosophical or political beliefs) prevent them from receiving a vaccine. 
  • Exemptions and reasonable accommodations must be initiated by the employee by submitting a request for exemption form (see above for appropriate form). All such requests will be handled in accordance with applicable laws and regulations and with UVM’s Policies and University Operating Procedures (UOPs) including, but not limited to, Procedural Guidelines for Disability Accommodation for Employees and UVM’s Equal Employment Opportunity/Affirmative Action Policy Statement.
