Completion of this form is only necessary in the event that a student/SAS Specialist requests additional flexibility beyond the default standards afforded by the flexibility with attendance and assignment extension accommodations detailed below.

The above-named student has been approved an Attendance Flexibility Accommodation and/or Assignment Extension Accommodation.

The Attendance Flexibility Accommodation provides modification to an attendance policy due to exacerbation of a disability or a disability-related condition.

Per this accommodation, SAS has determined that the student is allowed one additional absence beyond the stated attendance policy without penalty, provided that the student is required to notify the instructor in advance of utilizing this accommodation.

The student has either met the attendance limit or believes additional flexibility is necessary due to a disability-related reason. The student has contacted SAS, who has considered the request with you to evaluate the extent to which attendance is critical to the essential learning objectives of a class, and to help decide on whether additional Attendance Flexibility can be reasonably implemented. As such, SAS has guided you through a discussion of the following points:

1. What does the class description and syllabus say about attendance?
2. How is the final class grade calculated? Is attendance factored into the final grade?
3. Is the attendance policy consistently applied? (i.e.., Have there been any exceptions made to the policy for non-disabled students, such as for athletic travel or religious observances? If so, then these exceptions must also be granted to students with disabilities.)
4. Is there significant interaction between the instructor and students, and among students? If so, how much?
5. Do student contributions and participation in class constitute a significant component of the learning process? (i.e., discussion, presentations, role play, group work)
6. To what degree does a student’s failure to attend class compromise the educational experience of other students in the class?

Based on your responses above, please indicate the following:

1. FLEXIBILITY WITH ATTENDANCE
This should not be a restatement of the number of absences allowed for all students in the course). Be as specific as possible; please do not use vague phrases such as ‘flexible’, ‘to be determined’, or ‘open’.

When a student needs to utilize this accommodation, they are responsible for notifying their instructor in a timely manner. ”Timely” is defined as “as soon as possible.” There may be some conditions and/or circumstances in which a student is not able to contact their instructor prior to the class. Please note: At no time are students required to present documentation to instructors to justify a disability-related absence.
2. ASSIGNMENT EXTENSION

The Assignment Extension Accommodation provides extra time for out-of-class assignments or take-home exams due to an exacerbation of a disability or a disability-related condition. The student is eligible for a 1-day (24-hour) extension to a limit of two out-of- class written assignments, and the student is required to notify the instructor in advance of utilizing this accommodation. The student has either met the number of extensions or believes additional flexibility is necessary due to a disability-related reason.

When a student needs to utilize this accommodation, they are responsible for notifying their instructor in a timely manner. ”Timely” is defined as “as soon as possible.” There may be some conditions and/or circumstances in which a student is not able to contact their instructor prior to the class. Once the Agreement is emailed to the student, it become active.
