Use the FAMIS Self-Service on-line system to submit all other service repair requests to Physical Plant.
The most efficient way to make requests of Physical Plant personnel is to use FAMIS. You can submit detailed repair requests, report electrical, plumbing or mechanical problems in your building, or request a pickup of surplus property and extra trash. You need a username and password to use FAMIS. If you are new to the system, please email Service Operations Support or call 656-2560 and you will be issued a username and password.Go

Effective March 2009, for
surplus pickup of appliances,
lab equipment or moveable equipment, a supplemental form is
required in
addition to the
on-line FAMIS Service Request. Departments must download
and complete this Surplus Disposal Form
for all appliances or lab
equipment regardless of value, and for all moveable equipment valued at
$5,000 or more. (This form does not apply to furniture and computer
equipment.) Complete form, make a copy. Attach one copy to
the
item or equipment to be picked up; send original copy to UVM Surplus
via campus mail, fax or email. For more information on this

Faculty and Staff who don't
have access to FAMIS may email Service

Support to report minor
problems.
On-campus students should notify the Residence Hall Fix-It desk for problems or conerns.
Complete
the Request
for Keys/Card Access Form and
fax a signed copy to the Lock Shop Fax
