It also enables you save your data with the form. This is especially helpful when you want to complete the form in more than one session or need to revise your data.
After OTT receives a disclosure form with the digital signatures of all co-inventors (see our complete instructions for digital signatures), the submission launches our formal technology transfer process.
To take advantage of features in our online Disclosure form, please follow the steps below.
1. Download and install the latest version of the free Adobe Acrobat Reader software.
2. Download a copy of the Disclosure Form and save it to your local hard drive.

Our website provides the option of an interactive Adobe Acrobat Pre-Disclosure form (available soon) that you can save and print with your data. Use the form to organize your information about your invention.
Again, this is an optional tool and is not required for the tech transfer process. It is also not a substitute for the full Disclosure form we require to launch the tech transfer process.
Because the Pre-Disclosure form has no official use, signatures are not required. You may submit a copy to OTT by email or bring it with you when you meet with OTT staff.
