Population-focused community health experience involving needs assessment, program development, case management, health promotion, disease prevention, and protection strategies. Opportunities for interdisciplinary collaboration to meet the needs of an identified population are provided across settings.

Upon completion of this course, the learner will be able to: 1. Implement appropriate health-related assessments for communities-at-risk using standard tools, reports, databases and key informants. 2. Develop, implement, and evaluate interventions that address health promotion, disease prevention, and protections strategies with communities using evidence-based practice. 3. Apply the concepts of community partnerships and interdisciplinary collaboration to address health disparities in populations. 4. Apply the ANA Standards of Clinical Nursing Practice to community/public health nursing care.
