Case History:

Ms. Bell, a 71 year old retired telephone operator, presents to her PMD with a five-month history of irregular postmenopausal bleeding. She has not been taking her hormone replacement therapy. On exam, she is moderately obese, appears pale. A vaginal exam shows no obvious local source for the bleeding. Lab studies show a hematocrit of 34, normal differential and platelet count.

A D&C produces bulky uterine curettings. Histological examination shows severe cytological and architectural atypia of the endometrium. Ms. Bell had a hysterectomy and bilateral salpingo-oophorectomy. The uterine cavity is filled with focally necrotic, pale, friable tumor that does not extend through the serosal surface.
