Forms and Submission Requirements

NOTE: You must submit all listed forms electronically to iacuc@uvm.edu as of 10/01/09.

We require scanned PDF format with appropriate signatures.

Initial Submission Guidance
  • Animal Use Protocol Form (revised 10/09/09 to take out personnel roster - It is important that you provide a protocol version date on the footer of your protocol.)
  • Personnel Roster (10/09/09 - roster was separated from full protocol to allow smoother roster updates. The roster is still required at initial submission.)
    • All Pain Levels
      • submit PDF version of form with vet, scientific review and PI signatures
      • submit PDF version of any other supporting materials (A grant is no longer required if protocol form has an Infoed Proposal Number included.)
    • Protocols Identical to Previously Approved Protocols
        • submit PDF version of form with vet, scientific review and PI signatures
        • submit PDF version of a signed memorandum The memorandum should explain that the protocol is identical to a previously approved protocol with regards to hypothesis, specific aims, and animal species and use. Reference the approved IACUC file number and protocol title in this memorandum.

    Subsequent Submissions Guidance
    • Animal Use Protocol Amendment (updated 09/18/09)
      • submit PDF version of the form with PI signature
      • submit PDF version of a revised Animal Use Protocol form. Submit the entire protocol not just the affected pages. Indicate on your revised protocol where the changes are and revise the protocol version date in the footer of your protocol.
  • Amendment Algorithm
    • Request for Change in Personnel (It is important that you provide a version date on the footer of your Personnel Roster form.)
      • submit PDF version of form with PI signature (If you are adding more than one person at a time, a separate form needs to be completed for every new addition to the roster.)
      • submit PDF version of updated Personnel Roster (10/09/09 - For older protocols that still have the roster embedded within the protocol, please use this new roster form the next time you request to change the roster and use this new roster form going forward.)
     
    Continuing Review Guidance
    • Request for Continuing Review Form - (An email notice along with this form is generated by RPO staff and forwarded to the investigator 2 months prior to study expiration. Do not complete the html form that is attached to the notice as it is not a "fillable" form. You must either print the attachment and complete with pen or typewriter or use this word version. We encourage you to use this word version and incorporate any information from the notice you received from RPO into this word form.)
      • submit PDF version of this form with PI signature
