CCL-listed Item Shipped to a Foreign Country

              Question #14:

Is the CCL-listed item to be shipped to a foreign country?

YES       NO

If it is determined, after consultation with the University of Vermont (UVM's) Sponsored Project Administration (SPA) that the CCL-listed item does not require an export license but may be shipped under a License Exception, UVM will identify a shipper to assist with the transaction.

Records related to overseas shipments must be stored both locally and centrally on campus. The PI, the Project Manager, or another person to whom this responsibility is delegated keeps copies of the description of the item and its use, along with any confirming shipping documents, with the project files. In addition, a copy of these documents must be provided to the Office of Sponsored Programs.
